Request for Child Care Fee Subsidy Reinstatement

The number of available care spaces is limited and we want to ensure that we can offer child care fee subsidy to those most in need. For this reason applications will be prioritized based on several factors, including applicants return to work and/or school dates. New applications for child care fee subsidy will be only be considered once all reinstatements have been procesed. By submitting the Request for Child Care Fee Subsidy Reinstatement, you certify that the information given is complete and accurate.

Please note: applications for applicants working outside the home will be given first priority.



Person living with you who is your spouse, common-law partner of more than 3 years, or the parent of the child(ren).



Copies of the following documents must accompany this application:

  • Proof of employment if you have changed employers (Letter from your Employer or recent paystubs), for both applicants
  • Updated Work or School schedule for both applicants, if available.  Please note:  you will be required to submit your next 2 paystubs as proof of hours worked once your application for reinstatement has been approved.
  • Any outstanding documentation that was requested prior to March 16, 2020


Please note: 

  • In order to confirm continued eligibility for child care fee subsidy, an Annual Review must be completed.  If your Annual Fee Subsidy Review is past due or due soon, we will be contacting you to set up an appointment.  Failure to attend this appointment will result in the loss of child care fee subsidy.
  • Reminder:  All applicants (and where applicable the applicant’s spouse) are required to file an income tax return annually in order to be considered for fee subsidy eligibility.

This information is collected under the legal authority of the Child Care and Early Years Act, 2014, for the purpose of determining or verifying a person’s eligibility to participate in a child care or early years program or service or to receive financial assistance under this Act. Section 71 (1) 2.
